What Makes This Support Different?

  • Family-Led: Your voice and priorities guide the plan

  • Strength-Based: We focus on what’s working and build from there

  • Needs-Focused: We look at the root causes, not just the symptoms

  • Collaborative: Decisions are made with you, not for you

Who This Helps

This service may help families when:

  • A child receives many services, but progress is limited

  • Behaviors feel unsafe or destabilizing

  • A child has repeated hospitalizations or out-of-home placements

  • Family needs go beyond what one system can provide

What Is Wraparound?

Wraparound is a team-based planning process for families with complex needs. Families choose who is on their team—such as relatives, friends, schools, or providers—to create a plan built around strengths and goals.

OhioRISE

OhioRISE (Resilience through Integrated Systems and Excellence) is a specialized Medicaid program that provides care coordination and intensive supports for children and youth with complex behavioral health needs. The goal of OhioRISE is to help families navigate services and ensure that the supports a child receives work together across home, school, and community settings.

Through OhioRISE, families are connected with a care coordinator who works alongside the child, caregivers, and providers to develop a plan of care based on the child’s strengths and needs. The care coordinator helps families access services, bring together professionals from different systems, and ensure that everyone involved is working toward the same goals.

Depending on a child’s needs, OhioRISE may help connect families with services such as intensive home-based treatment, mobile response and stabilization services, behavioral health respite, and other community-based supports.

Children and youth who are enrolled in Ohio Medicaid and have significant behavioral health needs may qualify for OhioRISE following a Child and Adolescent Needs and Strengths (CANS) assessment.

Families can find additional information about OhioRISE, eligibility, and available supports by visiting the OhioRISE website or speaking with their child’s provider.
